Dear Parent:


The Sevier County Health Department and the Sevier County School System will be partnering in the Fall to provide seasonal influenza (flu) vaccinations at your child’s school. If your child is uninsured, there is no cost to you for the flu vaccine. If your child is covered by insurance, including TennCare and private insurance, the Sevier County Health Department will file a claim with the insurance plan and receive reimbursement directly from the insurance plan. There is no out of pocket cost to you for the flu vaccines provided during this school flu vaccination clinic.

Health Department nurses will vaccinate children and school staff using the injectable (shot). Clinics will begin as soon as the vaccine arrives at the health department (usually middle to late fall).

If you would like for your child to receive the injectable flu vaccine, look for the consent form that will be sent home on the first day of school.  Fill out both sides of the consent form completely. Be sure to sign the form as this will be your permission for your student to receive the vaccine. You must fill out a separate consent form for each student you would like to receive the vaccine. If at a later date you change your mind and do not wish for your child to receive flu vaccine at the school, please notify the school prior to the clinic date! This service to the students and staff is being done to decrease the impact of seasonal flu in our communities and to decrease school absenteeism.

El Departamento de Salud del Condado de Sevier  y el Sistema Escolar del Condado de Sevier    
están colaborando para proporcionar la vacunación contra la influenza (antigripal) de temporada en la escuela de su hijo(a). Si su hijo(a) no tiene seguro, la vacuna antigripal no le costará nada. Si su hijo(a) tiene cobertura de seguro, incluyendo TennCare y seguro privado, el Departamento de Salud del Condado presentará la factura al plan de seguro para recibir el reembolso directamente del mismo. Ud. no paga ningún costo directo por las vacunas antigripales que se ofrecen durante esta clínica de vacunación antigripal en la escuela.

Enfermeras del Departamento de Salud vacunarán a los menores y al personal escolar utilizando inyectables (inyección). Se dará inicio a las clínicas tan pronto como la vacuna llegue al departamento de salud (por lo general de mediados a fines del otoño).

Si desea que su hijo(a) reciba la vacuna antigripal, por favor llene completamente ambos lados del formulario de consentimiento adjunto. Asegúrese de firmar el formulario pues así otorga el permiso para que su estudiante reciba la vacuna. Ud. tiene que llenar un formulario de consentimiento separado para cada estudiante que Ud. desee que reciba la vacuna. Si cambia su decisión en una fecha futura, ¡por favor notifique a la escuela antes de la fecha de la clínica! Este servicio se ofrece a los estudiantes y al personal a fin de disminuir en nuestras comunidades el impacto de la gripe de temporada y para disminuir el ausentismo escolar.
