This survey is your opportunity to share what parents need when it comes to educating their children. Your input is extremely important. The survey will be used by your school’s Title I Planning Team in developing the Parent Involvement Policy, School-Parent Compact, and Calendar of Parent Events. Thank you for taking the time to complete this survey.

Immunization for 7th graders
